Intended Audience: Mentors
This form is to be used when requesting an extension of the Michigan Virtual term to accommodate for a unique or extenuating circumstance. Extensions will ONLY be considered when applied for by the student's mentor. Applications MUST be submitted 5 days prior to the end of the term for consideration. NO extensions are available during the summer term. A $50 extension fee will be charged for all granted extensions.
Also, under “school approvals” add a confirmative question along the lines of:
“I understand that a $50 extension fee will be assessed if the extension application is accepted and an extension plan is put in place.
