Mentors: Special Populations - Updating a 504 Record

The "Mentor Special Pops: Updating a 504 Record" process outlines the steps required for mentors to update or create a Special Populations Record for students with a 504 plan or Individualized Education Program (IEP) within the Student Learning Platform (SLP). This process ensures that the necessary documentation is accurately maintained and up to date, aligning with the student's current educational needs.

Instruction

  1. First, login to the SLP using the applicable MENTOR account.
    Note: Affiliation users cannot complete the process of creating or updating special populations records. 
  2. Next, ensure the student has been identified as having a 504 or IEP flag within their SLP profile.
    Note: Records cannot be created for students who don’t have this designation. 
    • If the student has the "504" or "IEP" flag, proceed to step 3.
    • If the student DOES NOT have the "504" or "IEP" flag, follow steps to update their profile and then return to step 3. 
  3. Next, tap the Students tab and then select Special Populations Filter from the left navigation menu.
  4. At this point, review the list of students and their statuses. 
  5. Next, tap the "Special Populations Record link for a student.
  6. Now, scroll down the page to the "Documentation" section and then tap the Make a copy button.
  7. Finally, tap Browse and proceed to upload the current 504 document before selecting the Save button.
    Note: The document will save with the end of the current school year as the expiration date.
